Child ADHD Assessment for Parents (NICHQ Vanderbilt)
This is the same assessment scale paediatricians use to evaluate ADHD in children, developed by the American Academy of Pediatrics and NICHQ. It is intentionally more thorough than a quick screen. It considers attention, activity, common co-occurring concerns and how your child is functioning at school, at home and in relationships.

Before you start: answer based on your child’s behaviour over the past 6 months, compared to what’s typical for their age. This is validated for children aged 6 to 12. It is a screening tool, not a diagnosis, several sources of information (school, home, a clinician) are needed for that.

ADHD screen, by domain — the official NICHQ Vanderbilt criteria: 6 or more of 9 items scored Often/Very Often in a domain, plus at least one Performance item scored 4 or 5, counts as a positive screen for that domain.
Other screens on this form — the Vanderbilt also checks for a few commonly co-occurring or overlapping concerns. A positive screen here isn’t a diagnosis of anything, it just means it’s worth mentioning to whoever assesses your child.
